This is a custom nameplate that I have made for Cloud, but in theory it would work over any character, as long as you rename Cloud to whoever you're replacing.
To install, you need to edit your ui_character_db.bin
Find Clouds slot, which is entry 56, and navigate down to the numbers of 37-44. Here, you change whichever slot you'd like to have the nameplate over to a number 2, replacing the number one. If you'd like to replace his default skin, just change number 37's slot to a 2 instead of a 1. If you have extra skins for cloud, continuing downward you'll see 45-52. These have 0's but can also be changed to 2 to get a custom nameplate. If you want your namplate over slot 12, you change 49's 0 to a 2.
After you do all that, just drop the file in data/ui/replace/append/chr/chrn_11
